Liability Waiver and Photo Release Form
Please read this waiver and photo release in full. By registering an athlete for any Blackout Basketball class, camp, practice, or program, the parent or legal guardian acknowledges and agrees to the terms below.
Information collected on the waiver: athlete's name and date of birth; parent/guardian name, phone, address, and email; emergency contact and phone; and any medical conditions, allergies, or physical limitations we should be aware of.
Assumption of Risk
I understand and acknowledge that participation in any basketball class, camp, practice, or game organized by Blackout Basketball involves inherent risks, including but not limited to, the risk of serious injury, disability, or death. I voluntarily assume full responsibility for any injury or accident that may occur to me or my child while participating in any activities.
Release of Liability
I hereby release, waive, discharge, and hold harmless Blackout Basketball, its owners, directors, officers, employees, volunteers, contractors, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by me or my child, whether caused by negligence or otherwise, while participating in or traveling to or from any Blackout Basketball program or activity.
I further agree to indemnify and hold harmless the above-mentioned parties from any loss, liability, damage, or costs that may arise due to my or my child's participation.
Physical Fitness
I acknowledge that my child is physically fit to participate in sports-related activities, and I agree not to allow my child to engage in any activity beyond their physical limitations.
Medical Emergency Authorization
In the event of a medical emergency, I authorize Blackout Basketball staff or volunteers to secure necessary medical treatment for my child and agree to be financially responsible for any costs incurred as a result of such treatment.
Parent / Guardian Certification
I certify that I am the parent or legal guardian of the child named below and that I have the legal authority to sign this agreement on their behalf. I understand that by signing this document, I am giving up substantial legal rights on behalf of myself and my child, including the right to sue.
Photo & Video Release
By signing, I also grant permission to Blackout Basketball to use photographs and/or video footage of my child for marketing, publicity, and promotional purposes in print, digital, and online formats, including social media, without compensation.
Acknowledgement
I acknowledge that I have read this waiver and release of liability in its entirety, fully understand its terms, and agree to be bound by it freely and voluntarily, without any inducement.
